Who We Are
Corazon Hospice Care, is licensed by the Texas Department of Aging and Disability Services and CHAP accredited. Corazon Hospice Care provides end of life care for the terminally ill. We emphasize quality of life by striving to meet the physical, emotional, social and spiritual needs of our patients as well as the needs of their loved ones. We are committed to providing the highest quality services and believe the patient is entitled to be as free of pain and symptoms as possible, and to live his or her life in a comfortable and compassionate environment. Each patient is assigned an interdisciplinary team of caregivers, dedicated to helping patients and their families share time together in the warmth, comfort and privacy of their own homes or primary residence.
What We Do
Hospice is not a place; it is a concept of care for those who have life-limiting illnesses. Hospice affirms life. It neither hastens, nor postpones the end of life. It asserts the belief that through sensitivity, appropriate care, and the support of a caring and compassionate community, terminally ill patients and their families can begin to prepare, mentally and physically- to face the inevitable challenges of now and beyond. Hospice care promotes dignity and comfort for the patient and for his or her family. Hospice team members are supportive in a wide array of areas and services, including physical care, emotional and spiritual support and social service interventions. Patients and their family members are often able to focus on the very essence of their lives; those things that matter to them most. Corazon Hospice Care can: Pay for medications related to terminal illness. Provide wheelchairs, hospital beds, bedside commodes, or any other medical equipment needed. Provide care for patients at home, in assisted living facilities, in nursing care, or in residential care. Provide nursing care, bathing and dressing assistance, daily essential care, and homemaking services. Provide spiritual and emotional support, for the patient and for the family, as well as volunteer and bereavement support. Provide additional services that may be required to serve individual patient needs. Provide respite when family members and /or caregivers need time away.
